You have agreed to attend a airbrush tanning session(s) with a
Tan-A-Mania Techcian to achieve a natural looking UV-Free tan. For best results, please read and indicate that you understand and agree with the following information:

1. An airbrush tan, like any self-tanning product, it will not protect your skin from burning in the
 natural sun or in a UV tanning bed.

2. For best results, Shower, Shave & Exfoliate well DO NOT apply make up, moisturizers, or deodorant proir to your airbrushing session.

3. Then wait 8 to 12 hours after being airbrushed before showering. This will allow your airbrush tan to fully develop and will ensure the optimal results.

4. The Cosmetic Bronzers in the airbrush tanning solution may transfer to clothing or fabrics. The Bronzers are water soluble; and they will wash off in your first shower, and wash out of most Fabrics, Silk, Lycra, Spandex, Nylon, Wool or Leather can stain.

5.The airbrush technician is trained to provide a smooth even tan. However, due to normal skin variations, and session variables, it is possible you may occasionally find some areas of your tan slightly lighter than others. In most cases, these differences are minor and disappear after a shower

6. All the components of the airbrush tanning solution have been safely used in cosmetics and nutritional products for decades and have been proven to be safe for the skin. If you have had adverse effects utilizing self-tanning products or moisturizers in the past, you may experience similar effects with the airbrush system.
Please discuss this with your Tan-A-Mania Technician before proceeding.

7. We recommend that you hold your breath, during facial spraying. We make this recommendation, because most individuals would prefer not to breathe the mist.

8. If you have any history of asthma or other respiratory or medical conditions that could be aggravated by the use of the Airbrush system, please consult a physician before proceeding. Please advise your tanning technician of any past allergic reactions to DHA products, or other medical concerns.

9. It is our policy at Tan-A-Mamia tan that all customers are required to wear a bottom. We will not airbrush customers in the nude. ________ (initial).

10. All minors (less than 18 years of age) must have a parent or legal
guardian present and give written Consent to have an airbrush session. I, ____________________________________ give my Consent for ___________________________________________to receive an airbrush session from Tan-A-Mania Tan.

                I, the undersigned, understand and agree to comply with all instructions. I have been properly instructed prior to my Airbrush Session. I am using these services at my own risk. I hereby authorize the agent of Tan-A-Mania Tan to perform such tanning procedures as may be deemed necessary or advisable, and have provided them with the above information required. I acknowledge that the results of airbrush tanning do vary, and that there are no guarantees of specific results. I hereby relieve this establishment, as well as the Tan-A-Mania Technician, and will not hold them responsible, for any liability involved in the use of the tanning process. Tan-A-Mania and their agent are not liable for any injury to person(s) or property.
